INFORMATION FOR NEW BROCK DENTAL PATIENTS

Your first appointment otherwise known as the "new patient exam" is an initial baseline assessment and will consist of the following:

  • personal and medical questionaire

  • x-rays (bitwing and panoramic) and where required, additional x -rays to assess questionable areas

  • intra-oral examination including TMJ assessment, teeth classification, soft tissue and cancer screening, periodontal assessment (inflammation, bleeding, plaque and calculus deposits, recession) and parafunctional habits (attrition, bruxism)

  • extra-oral assessment of facial appearances like symmetry, profile of face and general physical shape

  • odontogram (recording of existing teeth present, fillings and types, particularities of the teeth within the mouth such as crowding, gaps, rotation and tipped teeth)

  • caries and filling assessment to identify carious teeth and teeth with poor, broken down fillings and those in need of crown coverage, root canal, and extractions

Throughout this exam, the patient will be able to follow along and ask any questions that arise. A mirror mounted above will allow the patient to see what the dentist is seeing and follow tooth by tooth and gum what the concerns are. The patient will be able to recognize the caries and periodontal concerns the dentist has pointed out. Lastly, the patient will spend time in the consultation room reviewing the findings, diagnosis, and address their chief complaints. The patient can then schedule their initial cleaning and basic operative treatment while the staff send out the full predetermination or estimates of work to be done ( if needed). This is a good time to ask the dentist and front staff any questions regarding your insurance coverage and the difference if any, you will need to pay out of pocket.

Please assist us by providing us with the following information:

  • x-rays (you may request them from your previous dental care provider)

  • a list of your current medications

  • if you have dental insurance, please bring the necessary information and forms. This will save time and allow us to help you process any claims.

Please alert our office of any medical conditions prior to undergoing surgery. For example, if you are taking antibiotics, blood thinners, heart medications, or have high blood pressure, artificial heart valves or joints, or rheumatoid fever, please ensure it is communicated to Dr. Say or Dr. Palencar before treatment commences.
